Five steps back

The Tablet

The pill is not the impressive part. The impressive part is that anyone can tell whether it works, because for almost all of human history nobody could, and confident people were wrong for thousands of years.

You take two and stop thinking about it

Your head hurts. You open a cabinet, shake out two tablets, swallow them, and go back to what you were doing. Twenty minutes later the pain is smaller. You do not notice the moment it goes, because you were not watching for it.

You did not read the box. You did not weigh anything. You did not consider whether this particular bottle was made correctly, and you would not have known how to check.

What you did was trust a stranger, at a distance, about a chemical you cannot see, on a matter affecting your own body. And you were right to. That is the part worth explaining.

Start with the object. Those two tablets contain a measured quantity of an active compound, and the next two in the bottle contain the same quantity, and so do the two in a bottle on a shelf on another continent.

That sameness is not a small thing. A dose that varies is a dose you cannot reason about. The entire structure of instructions on the box, take two, not more than six in a day, only means something if every tablet is interchangeable with every other one.

So somewhere there is a factory holding a tolerance you could not perceive, on an item that sells for pennies, and doing it a very large number of times without anybody clapping.

02 Inside you

It does not numb you

Here is the common misunderstanding. The tablet is not an anaesthetic. It does not deaden the nerve or dull your brain. It works further upstream than that.

When tissue is damaged or inflamed, your body manufactures signalling molecules called prostaglandins. They are part of how a sore thing announces itself: they turn up the sensitivity of nerve endings, drive inflammation, and help set fever.

The tablet blocks the enzyme that makes them. Cyclooxygenase, usually shortened to COX, is the machine that converts a fatty acid into those signals, and the drug gets in the way of its working end so the reaction cannot proceed. 1

So the pain is not being hidden from you. It is being un-manufactured. The message is not intercepted; it is never written.

Now the part that should unsettle you slightly. Nobody knew any of that for most of the time we were using it.

Aspirin went on sale at the end of the nineteenth century and was taken by an enormous number of people for decade after decade. The mechanism was not worked out until 1971, when John Vane showed that these drugs act by blocking prostaglandin production. 2

Think about that ordering. Seventy years of a medicine working, reliably, at scale, while the reason remained genuinely unknown. We were not waiting on understanding. We had something better and stranger: a way to be confident that it worked without knowing why.

Which raises the question this whole piece is really about. How could anyone possibly know that?

03 The hardest problem

Almost everything gets better on its own

Try to establish, from your own experience, that a headache remedy works. You take it, and later you feel better. That seems like evidence. It is very nearly worthless.

Headaches end. Colds pass. Most pain fluctuates, and people tend to reach for a remedy when it is at its worst, which is precisely the moment it is most likely to improve next regardless of what they do. Add that people feel better when they believe they are being treated, and that both patient and doctor want the treatment to have worked, and you have a machine for producing false confidence.

This is not a hypothetical failure. It is most of medical history. Bleeding, purging, and a long catalogue of confident, popular, useless and sometimes lethal treatments persisted for centuries, defended by intelligent people citing patients who got better. They did get better. That was never the question.

The fix is one of the most underrated inventions there is, and it is not a substance. It is a procedure.

In 1947 and 1948 the Medical Research Council ran a study of streptomycin for tuberculosis, and Austin Bradford Hill built it so that patients were assigned to treatment or to the comparison group by random numbers. 3 4 That single choice is the whole trick.

Randomising does something no amount of care can do by hand. It makes the two groups alike not only in the ways you thought to measure, but in every way, including the ones nobody had imagined. Age, diet, wealth, severity, sheer luck: all of it gets shuffled evenly across both sides. Whatever difference remains at the end is attributable to the treatment, because nothing else was allowed to differ systematically.

Add blinding, so belief cannot bend the result, and counting, so a small real effect can be told apart from noise, and you have a way to answer a question that unaided human judgement had failed at for thousands of years.

Notice what kind of thing that is. It is not a fact about the world. It is a method for finding facts about the world, and once you have it you can point it at anything.

That is why the tablet in your hand is trustworthy even though you know nothing about it, and even though for seventy years nobody knew how it worked. The trust is not in the chemist. It is in a procedure that has been run, published, argued over, repeated by people with every incentive to find fault, and survived.

The knowledge is not in anybody. It is in the arrangement: the trials, the journals, the regulators, the replication, the arguments. No single participant holds it. That is the same shape as the power grid and the shipping container, expressed in evidence instead of electrons or fruit.

04 The world

What that method bought

Once you can reliably tell what works, results compound, and the results here are the largest change in ordinary human experience there has ever been.

Life expectancy roughly doubled over the last two centuries. 5 Some of that was clean water and sanitation, which is a different story on this site. A great deal of the rest was knowing which interventions were real: which drugs, which surgeries, which practices, which advice to abandon.

And the change is not mainly about heroic cures. It is a very long accumulation of small correct answers, each one established by somebody willing to test rather than assume, most of them arriving as a slightly better version of something that already existed.

That is the least glamorous engine of progress and by some distance the most powerful one.

05 Ninety-three million miles away

You are swallowing sunlight

Follow the tablet backwards and it leaves medicine entirely.

The compound is mostly carbon and hydrogen, arranged in a specific shape. Those atoms did not come from a plant grown for the purpose. They came, for most modern drugs, from petrochemical feedstock: the same buried ancient biology that gets burned for fuel, taken apart and rebuilt into something useful instead.

Which means the molecule easing your headache is made of sunlight that landed on Earth before there were people, was caught by living things, was buried, and was then reassembled into a shape that fits an enzyme in your body.

And the reassembling took energy. Industrial chemistry runs on heat and pressure and electricity, which is to say it runs on the grid, which runs on the same handful of sources every other story here arrives at. 6

So the chain closes where all of them close. The Sun holds about 99.86 percent of the mass in this solar system and converts roughly four million tonnes of itself into energy every second. 7 8 A sliver of that reaches here. Life caught a fraction of the sliver. The ground kept some of it. We dug it up, took it apart, and built a molecule.

Two hundred years ago, a person with your headache had nothing that reliably worked, and no way to find out which of the things on offer did. Not because they were stupid. They were exactly as clever as you. They were missing a method, and a supply chain, and a factory, and a grid.

You are none of those things either. You are one person, on a small planet, near an ordinary star, holding a tablet you could not make, that works for reasons you did not have to learn, verified by a procedure you never ran, produced by people you will never meet.

And it works anyway. That is the thing I find hard to be pessimistic about.
